You need JavaScript enabled to view it   We will aim to send you the requested forms within 5 working days.  Forms can only be posted to the Leader of the club or group.  For any queries relating to the completion of these forms please do not hesitate to contact us either by phone or email as above.

KEY POINTS TO REMEMBER:

* Always use black ink

* Never use correction fluid

* Do not write outside of the boxes

* Do not complete the back page of the Disclosure Form

* Make sure you provide a full address history for the last 5 years with no gaps

* Don't forget to sign and date the declaration

*  Documents used as proof of ID must be dated within the last two months and must show your current address

* ID check cannot be carried out by a family member
